Tips for making an intervention request

Be prepared to provide the following information:

  • Child’s name and contact information
  • Important date(s)
  • The name(s) of witness(es) and contact information, if applicable
  • Facts, acts, comments/remarks or other indicators that relate to the violation of rights
  • Other remedies you have pursued for the same facts (for example, the Local Service Quality and Complaints Commissioner, the Québec Ombudsman, or a Tribunal)
  • The corrective measures you are seeking as a result of making an intervention request to the Commission
